The Sun claim he will tell the new Manchester United boss that he wants to leave the club permanently if he's not in his first-team plans.
The Belgium international is reportedly not prepared to go out on loan again after a torrid spell with Borussia Dortmund last term.
Januzaj joined the German giants temporarily last summer, but was recalled in January after failing to score in his six top flight appearances.
The 21-year-old is said to be worried that another loan deal will see him slip further down the first-team pecking order at Old Trafford, especially with the impending arrival of Henrikh Mkhitaryan.
The player will reportedly discuss his future with Mourinho when he returns to Carrington for pre-season training this week.
According to The Sun, United want to loan Januzaj out to another Premier League club for guaranteed first-team action.
But any deal is likely to include a clause allowing the Red Devils to recall him if he's not getting getting enough playing time.
Januzaj has made 50 appearances for United since his arrival from Anderlecht in 2011.
